How Digital PR Helped a Health Brand Build Organic Authority & Visibility

This case study tracks a two-year off-page SEO and digital PR growth journey for a healthcare client with a 17+ year-old domain that had historically been under-leveraged from an authority-building perspective.
From October 2024 through September 2026, we ran a consistent, always-on organic growth program combining digital PR, brand mentions, community engagement, and sponsored publisher placements. The strategy was designed to build durable topical authority in a highly competitive healthcare environment rather than relying on short-term link acquisition bursts.

Our Strategy & Approach
Starting in October 2024, we ran an always-on, multi-channel link building and digital PR program built around four pillars:
- Digital PR: Pitching data-led, newsworthy angles to relevant publications to earn editorial coverage and contextual backlinks.
- Brand Mentions: Securing and reinforcing brand citations across relevant industry content to build topical association and trust.
- Community Mentions: Value-first participation across community platforms, including 50 dedicated Reddit mentions placed in relevant threads and subreddits.
- Sponsored Posts – Placements on vetted, relevant publisher sites to diversify the link profile and accelerate authority growth.
This program has now run continuously for 24 months without a single drop-off period. In total, we have built 2,797 backlinks for the client since September 2024 – an average of roughly 115 new links every month, sustained across the full engagement.

Organic Traffic Growth
The clearest signal of the campaign’s impact is the client’s average organic traffic trendline. Starting from roughly 3,000 average monthly organic visits in December 2024, traffic climbed steadily through 2025 and into 2026, reaching approximately 12,000 average monthly organic visits by September 2026, an increase of nearly 4x over the course of the engagement. Growth was not a single spike; it compounded month over month as new backlinks, brand mentions, and community placements accumulated authority and referral pathways.

Backlink & Referring Domain Growth
Off-page authority scaled in lockstep with the traffic gains. The link building and digital PR work is directly reflected in the client’s current backlink and referring domain profile:
Figure 2: Backlinks and referring domains, current period vs. all-time (Ahrefs)
- Backlinks: 9,400 in the current tracked period (+1,400), against an all-time total of 28,900, meaning the campaign is directly responsible for a substantial share of the domain’s entire link history.
- Referring domains: 2,900 unique linking domains in the current period (+341), against an all-time total of 4,900, showing genuine diversity, not just volume, in the link profile.
Geographic Traffic Distribution
The link building and digital PR placements were concentrated in the client’s core target markets, and this shows clearly in the traffic-by-location breakdown. Canada and the United States together account for the large majority of organic traffic, with meaningful and growing contributions from the UK, the Philippines, and India:
Figure 3: Organic traffic by location (Ahrefs)
- Canada: 7,600 visits (+2,200), a 68.5% share of total organic traffic, backed by 915 ranking keywords (+164).
- United States: 2,900 visits (+858), a 26.0% share, backed by 399 ranking keywords (+3).
- United Kingdom, Philippines & India: Smaller but growing footholds, together contributing 448 visits and continued keyword expansion, evidence that the authority gains are extending demand beyond the core market.
AI Search Visibility
Beyond traditional organic rankings, the strengthened authority profile is now showing up in AI-powered search and answer engines. An increasingly important surface as users shift from classic search to AI assistants and overviews:
Figure 4: AI platform responses & AI Overview appearances (Ahrefs AI visibility index)
- 1,500 total responses across all AI platforms (+178), spanning 286 unique pages (+29).
- 688 appearances specifically within Google AI Overviews (+253), the fastest-growing segment across 183 pages (+48).
- Strong representation across Google AI Mode (610 responses), Perplexity (79, +29), Gemini (50, +28), and Copilot (10, −4), showing the domain is being recognised as a citable, trustworthy source by multiple AI engines, not just one.

Conclusion
This engagement shows what a consistent, multi-pillar off-page SEO strategy can do for a mature but under-leveraged domain. By combining digital PR, brand mentions, community engagement, and sponsored placements, and sustaining that mix for two full years.
We helped the client roughly quadruple their organic traffic, build a genuinely diverse backlink profile, and establish visibility not just in traditional search, but in the emerging world of AI-powered answer engines. The program remains active, and we expect continued compounding gains as the link profile and content authority keep maturing.
